The Silence of the Lambs (1991)
An acclaimed film, but I found it rather hackneyed, with so many of the usual elements that are put in to suspense thrillers to heighten the tension; for me they heighten the implausibility. A particularly nasty killer happens to be a transsexual, Jame "Buffalo Bill" Gumb, played by Ted Levine.
We see a brief full frontal shot, though I think this was achieved by tucking rather than surgery.
